HIPAA Notice of Privacy Practices (NPP) Gauld Foot & Ankle

Effective Date: February 16, 2026 THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

This notice governs Gauld Foot & Ankle, a subsidiary clinical division of Gauld Health.

A. Information Sharing & Clinical Coordination We share Protected Health Information (PHI) within the Gauld Health network as necessary for care coordination, payment, and business operations.

B. 2026 SUD Alignment Rule Gauld Foot & Ankle adheres to the 2026 SUD Alignment Rule. Records regarding Substance Use Disorder (SUD) are treated with heightened confidentiality and generally require a qualifying court order or specific written consent for use in legal proceedings.

C. Mobile Privacy & Consent-First Policy Participation in the Gauld Foot & Ankle text messaging program is 100% optional and requires explicit patient opt-in.

Consent Requirement: No text messages will be sent to your mobile device before your affirmative consent is captured and recorded.
No Third-Party Sharing: No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.
Subcontractor Support: Information sharing to subcontractors in support services, such as customer service, is permitted.
SMS Data Protection: All categories of our data sharing exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties, excluding aggregators and providers of the Text Message services.

D. Redisclosure Warning You are notified that PHI disclosed to authorized third parties with your permission may be subject to redisclosure by the recipient and may no longer be protected by federal HIPAA regulations.
